Anonymous Report Form: When to Use This Resource


Our school is committed to providing a safe and supportive environment for every student. With this goal in mind, we have added an Anonymous Report Form to our school website. This form (click on the button above) is available for students and families who need to share a concern anonymously.


Whenever possible, we encourage concerns to be addressed through our regular communication channels:

1. Start with the Teacher
For classroom, peer, academic, or day-to-day concerns, please begin by communicating directly with your child’s teacher. This allows the person closest to the situation an opportunity to understand and address the concern.

2. Connect with Administration
If the concern cannot be resolved with the teacher, or if it involves a broader school concern, families are encouraged to contact a member of the school administration.

3. Use the Anonymous Report Form When Direct Communication Is Not an Option
This form is intended for situations in which an individual does not feel comfortable, does not feel safe, or is unable to report the concern through the usual channels. It may also be used when anonymity is important to the individual reporting the concern.


Please Use This Form For:

  • Concerns about student safety or well-being
  • Bullying, harassment, or repeated concerning behavior
  • Reporting concerning or risky behavior on social media
  • Situations where speaking directly with the teacher or administration is not a reasonable option
  • Concerns that an individual may be hesitant to report openly

Please Keep in Mind

Anonymous reports are taken seriously; however, providing specific information helps the school determine the appropriate response. Because the report is anonymous, our ability to ask follow-up questions or provide information about the outcome may be limited.


Our goal is always to encourage open communication, problem-solving, and partnership with families while ensuring that everyone has a way to speak up when traditional communication channels are not an option.
